In-Processing FORMS

A staff member, volunteer, or veteran will help you with in-processing. Please do not fill these out alone. 

If you do not want or need access besides the days that we are open you do not need to go to step 2 for payment. Step 2 is only for 24/7 access to the facility. 

We are open to veterans every Thursday night from 7pm - 9pm and every Sunday to veterans and volunteers from 9am - 5pm.

the severed souls are a veteran 501(c)(3) organization that has done so much for helping us improve our facility here at OCB and also help countless veterans in our community. 

Eligibility:

1. 32 hours of participation in Sunday Shop Days (4 days) participating in veteran therapy or volunteering
2. Current liability form completed
3. Detailed facility tour explaining start up and shut down procedures. 

Veteran Access Benefits:

1. 24/7 access to facility and use of all tools and shop resources (explained in personal tour of facility)

Volunteer Access Benefits:

1. 24/7 access to facility and use of all tools and shop resources (explained in personal tour of facility)
   
2. Use of facility is based on a 50/50 volunteer / reward rate. Meaning: we ask that you volunteer just as much as you use the facility for personal use. 
3. The facility is for veterans mental health and their mental health and usage is priority. 
4. Volunteer access can be cancelled at anytime.

Once step 1 & 2 are completed you will be given a tour and issued a security clearance card.

This card will get punched once a year when you fill out an annual liability waiver.

This card may be asked to be shown by staff or a veteran at anytime so please keep it on your person when in the facility.
